DOG ADOPTION CONTRACT Kennyacres 1438 Q Rd Central City, NE 308-624-0318 |
The purpose of this contract is to create and/or memorialize the adoption and transfer of ownership of one dog from Kennyacres, herein called Rescuer, to________________________________________________________________________herein called Adopter. This transfer took place on ____/____/________. The dog is described as follows: Name:________________________ Breed:________________________ Age (if known):_________________ Sex:______________ Color:_______________________________________________________ Rescuer obtained this dog from another source under conditions such that the particulars of age, breeding, registration, prior training, health, etc are unknown and untraceable. Therefore Rescuer makes no guarantees or warrantees of any kind on this dog. Rescuer is of the opinion that this above described dog appears to have a temperament and personality suitable for a family pet and companion if provided good training and handling by the Adopter. Rescuer will provide advice as the Adopter may request to deal with any problems of adjustment and training. Adopter has paid the sum of $______________ as an adoption fee to reimburse Rescuer for expenses associated with the cost of rescue and rehabilitation. Transfer of this dog is in consideration of the following promises by the Adopter. These promises are material and are the essence of the contract. 1. The Adopter will provide the dog with basic needs such as fresh water and food at all times. 2. The dog will have shelter sufficient to be able to withstand the elements, especially during the winter and summer months. 3. The adopter will prevent illness by providing necessary vaccinations and veterinary care. 4. The Adopter will provide a loving home for the life of the dog. 5. If at any time the Adopter cannot care for the dog the Adopter will contact the Rescuer to relinquish the dog back. The adopter will NOT be refunded for any expenses related to the dog, including the adoption fee.
